Abstract
Introduction: In recent years, the management of blunt abdominal trauma has changed significantly, with increasing acceptance of nonoperative treatment for injuries affecting organs such as the spleen, liver, and kidneys. Objective: To discuss the role of conservative treatment in blunt abdominal trauma, highlighting indications, strategies, and outcomes across different abdominal organs. Methods: A narrative literature review was conducted using articles retrieved from PubMed, covering the period from 2010 to 2024. The search identified 160 publications, of which 20 were selected for discussion according to the study criteria. Results: Advances in trauma care and diagnostic imaging have facilitated the adoption of less invasive approaches, particularly in hemodynamically stable patients. Nonoperative management may preserve organs and reduce morbidity, unnecessary surgical procedures, and hospital costs when careful patient selection and monitoring are available. The management strategy depends on the injured organ, clinical findings, hemodynamic status, and access to diagnostic and therapeutic resources. Final considerations: Despite growing support for conservative treatment of blunt abdominal trauma, differences remain among institutions and countries regarding patient selection, monitoring protocols, and therapeutic strategies. Individualized assessment and ongoing clinical surveillance are therefore essential for safe decision-making.
